  1. Complete the Repair Form below (If unable to print, include a handwritten note with the information requested on the form)

  2. Package Your Devices (Securely pack your hearing aids along with the completed Repair Form or note)

  3. Ship to Our Mailing Address (Hear & There Audiology, 236 Loto St Unit 137, Eagle Point, Oregon, 97524) 

  4. We will send you a confirmation email with next steps as soon as devices arrive

*Mail-in repair services are for the mechanical repair of the devices onlyNo clinical advice, reprogramming (except to restore prior settings), or recommendations will be given. If you live in Colorado or Oregon, reprogramming requests can be completed for an additional fee.
